Faktor Penyebab Rendahnya Pelaporan Insiden Keselamatan Pasien di Rumah Sakit: Literature Review

Abstract

Pelaporan masalah keselamatan pasien sangat penting dalam sistem perawatan kesehatan, karena pelaksanaannya di rumah sakit belum optimal. Sulit untuk mengidentifikasi kesalahan karena rendahnya tingkat pelaporan begitu banyak kendala yang dialami oleh petugas kesehatan dalam pelaksanaannya keselamatan pasien di rumah sakit. untuk mengetahui faktor yang menyebabkan rendahnya pelaporan insiden keselamatan pasien di rumah sakit. Penelusuran artikel dilakukan melalui empat database PubMed, Science Direct, EBSCO/CINAHL, dan Garuda dengan menggunakan kata kunci Healthcare professional OR Doctors OR Nurse OR Nurses AND event reporting OR patient safety reporting OR incident reporting OR adverse event OR medical error AND Hospital. Artikel teridentifikasi n=1.132, tetapi hanya 12 artikel yang sesuai dengan kriteria inklusi tersisa 8 artikel. Pelaporan insiden keselamatan pasien di rumah sakit yaitu hambatan pada individu itu sendiri takut dihukum, disalahkan, dinilai tidak kompeten, merusak reputasi, waktu, tidak ada niat karena tidan ada umpan balik, dan menerima konsekuensi hukum serta intimidasi apabila melaporkan insiden. Kurangnya pendidikan, pengetahuan, pelaporan tidak dianggap sebagai kewajiban dan pelatihan tentang tatacara melaporkan insiden, serta kurangnya persiapan dan keterampilan komunikasi dalam melaporkan insiden. penyebab dalam melaporkan insiden tentunya menghambat peningkatan keselamatan pasien sehingga diperlukan upaya untuk mengatasinya. Komitmen para pembuat kebijakan memainkan peran penting dalam meningkatkan sistem pelaporan insiden keselamatan pasien.
