Abstract

Prevalensi pasien gagal jantung yang menjalani rawat inap meningkat setiap tahun. Perawatan keperawatan berbasis bukti diperlukan untuk meningkatkan kemampuan pasien dalam merawat diri dan mengurangi insiden re-hospitalisasi. Penelitian ini bertujuan untuk menerapkan hasil penelitian pendidikan kesehatan terintegrasi dengan konseling dalam bentuk Model "PrOMiSe". Penelitian ini menggunakan metode penerapan keperawatan berbasis bukti dengan menganalisis masalah berdasarkan PICO. Pencarian literatur dilakukan melalui beberapa database, termasuk Cinahl, Proquest, Ebsco, dan science direct dengan kata kunci yaitu pendidikan perawat, gagal jantung kongestif, intervensi perawat, pengetahuan, perilaku merawat diri. Intervensi dilakukan pada sepuluh responden selama enam minggu. Hasil aplikasi menunjukkan bahwa indeks pengetahuan rata-rata dan indeks perawatan diri mengalami perbedaan yang signifikan setelah menerapkan model tersebut. Kesimpulannya, penerapan Model "PrOMiSe" pada pasien gagal jantung memiliki efek peningkatan pengetahuan dan kemampuan merawat diri sehingga berdampak pada peningkatan status kesehatan dan mengurangi peluang re-hospitalisasi.
